Design-Builder’s Application for Payment includes integrated worksheets for the payment summary, schedule of values for lump sum work, schedule of values for unit price work, and a stored materials summary.  This document is similar EJCDC C-620 (for design-bid-build) but is specific to design-build project delivery. 

EJCDC includes the American Society of Civil Engineers, the American Council of Engineering Companies, the National Society of Professional Engineers, and the participation of more than fifteen other professional engineering design, construction, owner, legal, and risk management organizations.
